I’ve always used the diffuser plate myself. I just felt more comfortable using it. But, sometimes it could get in the way if hanging long racks of ribs or a big brisket. There’s certainly no harm in not using it, the PBC people hang food all the time without a diffuser.

Yes and no. I leave out the diffuser plate, but I use a Hunsaker Vortex charcoal basket. The vortex does kind of the same thing as the diffuser plate, but sits slightly below where the diffuser plate would be. I don't like stuff dripping directly onto charcoal, and the vortex sizzles it all away on the hot metal before it can drip on down.
