It costs you absolutely nothing. To date, Amazon has donated $144,958,582.84 to all charities, and that isn’t chump change. I fill the cart via the app and then open a browser and go to smile. Alternatively you could bypass the app (which I really only use when they have app only deals) and just make a button on your home screen that opens smile.amazon.
